Qualified students with disabilities (SWD) that have been assigned academic adjustments and/or accommodations are required to identify their needs to faculty members by presenting an accommodation memorandum from the SFSWD Director. The memorandum lists all assigned classroom/testing accommodations. Faculty members are responsible to provide the assigned classroom and testing academic adjustments and/or accommodations unless resources are not available. If assistance is needed, please notify the SFSWD staff @ ext. 8347 or e-mail Pamela.Ervin@lamar.edu. Please remember that information contained in the accommodation memorandums is CONFIDENTIAL and should not be disclosed to a third party without the expressed consent of the student (see A.D.A. Title 1 at 42 USC ss12112(d)(3) & (4); 29 cfr ss 1630.14-1630.16). Any questions should be referred to the Office of Services for Students with Disabilities at ext. 8347 or Callie.Trahan@lamar.edu.
